PLAN YOUR CONSULTATION AT THE IRIS OPHTHALMOLOGY CLINIC

The IRIS Ophthalmology Clinic team will be happy to welcome you in a calm and safe environment during your preoperative assessment, which will last approximately two (2) hours. In order to optimize your visit and allow our professionals to respond to your needs, we invite you to read the following instructions carefully:

BOOK AN APPOINTMENT

STEP 1: MAKE AN APPOINTMENT

Submit an appointment request and complete the questionnaire which will be sent to you by email in order to speed up the admission process and the opening of your file. We suggest that you complete your medical questionnaire online before your virtual consultation or your preoperative evaluation at the clinic. The information collected will allow our professionals to identify your visual needs and assess your candidacy for surgical vision correction.

STEP 2: SEND US A SUMMARY OF YOUR EYE-VISUAL FILE

With your consent, your vision professional may send us a summary of your optometric file, before your virtual consultation or your preoperative evaluation at the clinic.

Contact your optometrist to ask him to fill out the preoperative referral form, available online for professionals, in the “Referrers Section”. Information can be sent to the IRIS Ophthalmology Clinic:

If you are unable to reach your optometrist, send us your most recent prescription for glasses or contact lenses. This information will allow our professionals to assess the surgical correction alternative that best suits your vision.

STEP 3: STOP WEARING YOUR CONTACT LENSES

In order to ensure the accuracy of the preoperative measurements, please plan to stop wearing your contact lenses completely before the evaluation:

  • Soft contact lenses, daily wear (removed every evening): at least 7 days off.
  • Soft lenses, extended wear (worn overnight): at least 14 days off.
  • Soft toric lenses (correcting astigmatism): at least 14 days off.
  • Semi-rigid gas permeable lenses: at least 1 month off for every 10 years of wearing.

If you are not able to function with your glasses during the suggested period, the clinic will offer you a partial evaluation as a first step.

  • Ideally, plan to remove your contact lenses at least 24 to 48 hours before the partial assessment.
  • If you are planning surgery, you may need to stop wearing contact lenses for the recommended period, for the second full preoperative assessment.

STEP 4: PROVIDE A DESIGNATED ACCOMPANIMENT

Dilatation drops may be used to fully assess the health of your eyes.

It is strongly recommended that you have a designated companion as you will not be able to drive immediately after the assessment. The effect of the dilation is approximately 4 to 6 hours.

  • Dilation can cause blurred vision and increased sensitivity to light. Bring sunglasses to promote your comfort.
  • In order to limit the number of people circulating inside the clinic:
    • Allow only one person to accompany you during your visit;
    • Provide a designated person to look after any young children;

    • Respect the time of your appointment. If you arrive early, stay outside the clinic until the scheduled time for your appointment.
  • If you are unable to be accompanied or to dilate, the clinic will offer you a partial assessment first.

STEP 5: BRING YOUR IMPORTANT DOCUMENTS

To facilitate the opening of your file, remember to bring your useful documents during your consultation:

  • Your medication and allergy list, if applicable.
  • Your Quebec health insurance card (RAMQ).
  • Plan a method of payment to pay the consultation fees.
    • The cost of a consultation will be communicated to you upon request.
    • The IRIS Ophthalmology Clinic undertakes to deduct the consultation costs from the cost of the surgery, if it is performed within 12 months of the date of the evaluation.

STEP 7: LEARN ABOUT THE INFECTION PREVENTION STEPS 

 

Check your health and symptoms:

If you have unusual symptoms we will invite you to postpone your visit to a later date.

Wearing a medical mask:

  • The medical staff will wear a mask during your evaluation;
  • Wearing a mask is recommended in certain circumstances:
    • If you have symptoms (e.g.: fever, cough, sore throat, nasal congestion)
    • If your health makes you more vulnerable (e.g.: immunocompromised person, pregnant woman, elderly person, etc.)
  • The clinic will provide you with a disposable mask upon your arrival, if needed.

Hand hygiene:

  • Upon arrival, disinfect your hands and frequently used items (smartphone, electronic tablet, etc.).
  • Hand disinfection products will be available in strategic locations throughout the clinic.

Limitation of non-essential contacts:

  • In order to limit the number of people circulating inside the clinic:
    • Allow only one person to accompany you during your visit;
    • Provide a designated person to look after any young children;
    • Respect your appointment time. If you arrive early, stay outside the clinic until your scheduled appointment time.
