How to prepare well for your teleconsultation

You have obtained an appointment for a teleconsultation. Here is the practical information to help you prepare for the meeting with one of our professionals.

PLEASE LEARN ABOUT THE 4 STEPS

STEP 1: COMPLETE THE ONLINE MEDICAL QUESTIONNAIRE BEFORE YOUR CONSULTATION

In order to speed up the admission process and the opening of your file, we suggest that you complete your medical questionnaire online, before your virtual consultation or your preoperative assessment at the clinic.  The information collected will allow our professionals to identify your vision needs and assess your candidacy for surgical vision correction.

  • The questionnaire is divided into sections. It should take you less than ten (10) minutes to answer all the questions.
  • Answers will be transmitted in a secure manner, ensuring confidentiality and protection of your information.
  • A copy of your questionnaire will be emailed to you once completed.

Get started!  Complete the questionnaire in one simple step, by clicking here:

STEP 2: SEND US A SUMMARY OF YOUR OCULO-VISUAL FILE

With your consent, your vision professional may send us a summary of your optometric file, before your virtual consultation or your preoperative assessment at the clinic.

Contact your optometrist to ask him or her to fill out the Preoperative referral form, available online for professionals, in the “Referents Section”.  Information can be sent to the IRIS Ophthalmology Clinic:

By fax: 450.688.9516 or 1.877.674.8256

By email: [email protected]

If you are unable to reach your optometrist, send us your most recent prescription for glasses or contact lenses.  This information will allow our professionals to assess the surgical correction alternative that best suits your vision.

STEP 3:  READ THE SURGERY PREPARATION GUIDE

Get information and answers to your questions by browsing the IRIS Ophthalmology Clinic’s website before your consultation with one of our professionals.

Click to download!

We also encourage you to download the Surgery preparation guide most suitable for your needs and expectations:

  • If you are between 18 – 40 years old, we suggest you read the Guide to Ultralasik Laser Surgery.
  • If you are between 40 – 54 years old, we suggest you read both guides.
  • If you are 55 and over, we suggest you read the Phacorefractive Surgery Guide.
