Important recommendations before your screening appointment

1. Complete the online medical questionnaire

To help expedite the registration process and the creation of your medical file, we recommend that you complete your online medical questionnaire before your screening appointement at the clinic. The information you provide will help our healthcare professionals identify your visual needs and assess your candidacy for vision correction surgery.

 

• The questionnaire is divided into sections and should take less than ten (10) minutes to complete.

 

• Your responses will be transmitted securely to ensure the confidentiality and protection of your personal information.

 

• Once you have completed the questionnaire, a copy of your responses will be sent to you by email.

2. If possible, allow sufficient time to stop wearing your contact lenses

To ensure the accuracy of preoperative measurements, please plan to completely stop wearing your contact lenses before the evaluation:

 

  • Soft contact lenses, daily wear (removed every night): at least 7 days without wear.
  • Soft contact lenses, extended wear (worn overnight): at least 14 days without wear.
  • Soft toric contact lenses (for astigmatism correction): at least 14 days without wear.
  • Gas-permeable rigid contact lenses: at least 1 month without wear for every 10 years of use.

* If you are unable to stop wearing your contact lenses for the recommended period before your screening appointment, it will be possible to postpone this period of discontinuation until before your complete preoperative assessment. However, some imaging tests will need to be repeated to ensure the accuracy of the measurements and confirm your eligibility for surgery.

3. Lubricate your eyes with artificial tears

Before your consultation, it is recommended to lubricate your eyes with artificial tears 4 times a day. Lubrication helps facilitate the measurements taken during your consultation. Ideally, it would be beneficial to lubricate your eyes for a period of two weeks (14 days).

If the time between booking your appointment and the date of your visit is less than two weeks, simply lubricate your eyes 4 times a day until the date of your appointment.

4. Bring your important documents

To facilitate opening your file, remember to bring the following documents to your consultation:

 

• Your list of medications and allergies, if applicable.

• Your Quebec Health Insurance Card (RAMQ).
