Important recommendations before your follow-up appointment

To help optimize your visit to the clinic and enable our professionals to better meet your needs, we invite you to carefully read the following instructions :

Step 1

If applicable, arrange for a designated driver or companion

  • If you have been informed that pupil dilation will be required during your appointment to allow for a comprehensive examination of your ocular and visual condition.
  • If you feel that your vision is not adequate to drive safely.
  • If you require assistance due to a particular situation.

The effects of pupil dilation typically last approximately 4 to 6 hours.

  • Pupil dilation may cause difficulty focusing at near distances, blurred vision, and increased sensitivity to light.
  • Driving is not recommended after pupil dilation.
  • Bring a pair of sunglasses for your comfort.

STEP 2

IF APPLICABLE, PLAN FOR THE CONSULTATION FEES

Postoperative follow-up appointments related to surgery performed at the IRIS Ophthalmology Clinic are generally provided at no charge for up to two (2) years following the date of your surgery.

Consultation or follow-up fees may apply in certain situations. For example:

  • More than one (1) year has passed since your preoperative evaluation at the IRIS Ophthalmology Clinic and you have not yet undergone surgery.
  • More than two (2) years have passed since your surgery at the IRIS Ophthalmology Clinic.
  • You underwent surgery at a clinic other than the IRIS Ophthalmology Clinic.

Please contact our Customer Service team to determine whether fees apply to your situation.

STEP 3

IF YOUR SURGERY WAS PERFORMED MORE THAN TWO (2) YEARS AGO, PLEASE HAVE A SUMMARY OF YOUR MOST RECENT OPTOMETRIC EXAM SENT TO US.

The IRIS Ophthalmology Clinic primarily provides vision correction surgery and related postoperative follow-up care for a period of two (2) years.

Please consult your trusted optometrist for any other eye care needs. For example:

  • Your routine annual eye examination.
  • Any urgent eye concern or symptoms that arise more than two (2) years after your surgery at the IRIS Ophthalmology Clinic.

If your optometrist determines that your condition requires a referral to the IRIS Ophthalmology Clinic, please ask them to complete and send us a Postoperative Evaluation Form. This form is available online for eye care professionals in the Referring Providers section.

The completed form may be sent to the IRIS Ophthalmology Clinic:

By fax: 450.688.9516 or 1.877.674.8256
