During this interview, which lasts approximately 40 minutes, a qualified professional will be happy to discuss your oculo-visual history, your needs and your expectations. The professional will take the time to explain the various vision correction alternatives that may be offered to you by the ophthalmologist, as well as the eligibility criteria for surgery and the steps to prepare for it.

* The virtual consultation cannot replace the preoperative evaluation on site at the IRIS Ophthalmology Clinic. Using advanced imaging techniques and a comprehensive oculo-visual examination, our professionals will validate that you meet the eligibility criteria for surgery.

You will receive instructions via email to log in when the day arrives.

HOW MUCH DOES THE VIRTUAL CONSULTATION COST?

  • The cost of a consultation will be communicated to you on request.
  • The IRIS Ophthalmology Clinic undertakes to deduct the costs of the consultation from the price of the surgery, if it is performed within 12 months of the date of the assessment.

STEP 2: COMPLETE THE ONLINE MEDICAL QUESTIONNAIRE SENT TO YOU BY EMAIL BEFORE YOUR CONSULTATION

In order to speed up the admission process and the opening of your file, we suggest that you complete your medical questionnaire online, before your virtual consultation.  The information collected will allow our professionals to identify your vision needs and assess your candidacy for surgical vision correction.

  • The questionnaire is divided into sections. It should take you less than ten (10) minutes to answer all the questions.
  • Answers will be transmitted in a secure manner, ensuring confidentiality and protection of your information.
  • A copy of your questionnaire will be emailed to you once completed.

STEP 3: SEND US A SUMMARY OF YOUR OCULO-VISUAL FILE

With your consent, your vision professional may send us a summary of your optometric file, before your virtual consultation or your preoperative assessment at the clinic.

Contact your optometrist to ask him or her to fill out the Preoperative referral form, available online for professionals, in the “Referents Section”.  Information can be sent to the IRIS Ophthalmology Clinic:

If you are unable to reach your optometrist, send us your most recent prescription for glasses or contact lenses.  This information will allow our professionals to assess the surgical correction alternative that best suits your vision.

STEP 5: FOLLOW THE PROCEDURE TO ACCESS YOUR ONLINE APPOINTMENT?

1.  The day before your appointment:

  1. Make sure you have a smart device equipped with an audio camera, as well as a reliable internet connection (laptop, smartphone).
  2. If necessary, make sure you have someone available to assist you during the connection process.
  3. Download and install the GoToMeeting app on your device using the following instructions.

 

2. On the day of the appointment:

  1. An email will be sent to you a few hours/days before your teleconsultation meeting.
  2. Make sure you are in a quiet, private and confidential environment;
  3. Have your Quebec Health Insurance card ready to validate your identity at the start of the consultation.
  4. Sit in front of your device, a few minutes before the scheduled consultation time;
  5. Go to the GoToMeeting link sent to your email address and click on the GoToMeeting link. The meeting will begin on its own. It will be initiated by a member of the IRIS Ophthalmology Clinic.
  6. If you don’t see the email with the GoToMeeting link, make sure to check in your junk folder.
